Affiliation FF Sports Pour Tous: Forms to complete<\/h2>\n

1st case: you are the president of an association<\/h3>\n
    \n
  1. Demande d\u2019Affiliation d\u2019association<\/a><\/strong>
    \nComplete the form and attach all requested documents<\/li>\n
  2. Demande de licence Cadre Technique<\/a><\/strong>
    \nIf you are the president of the association, write in “je suis aussi dirigeant” on the form Cadre technique<\/li>\n
  3. A cheque for the amount of the Affiliation + Licence Cadre technique fees<\/strong><\/li>\n<\/ol>\n

    2nd case: you are an independent, autoentrepreneur, lib\u00e9ral<\/h2>\n

    We will consider that you have several roles: you are the “structure” joining the FFEPMM, the manager, and the cadre technique. Therefore, you indicate your name and personal information on all forms.<\/p>\n

      \nComplete the form and attach your Kbis or the attestation Siret ou Siren + a copy of your CQP ALS diploma (if you have already received it. Otherwise, tick the box “En cours de formation” in the part labeled “Cadre technique principal”)
      \nIf you do not wish for your personal details to appear on the FFEPMM Sports Pour Tous website, please state it on a post-it.<\/li>\n
    2. 1st demande d\u2019agr\u00e9ment f\u00e9d\u00e9ral<\/a><\/strong>
      \nDo not attach the “documents \u00e0 fournir”<\/li>\n
    3. Demande de licence Cadre Technique<\/a><\/strong>
      \nMake sure that you have signed twice (once on the front, the second on the back)
      \nWrite in on the Cadre technique form \u00ab je suis aussi dirigeant \u00bb<\/li>\n
    4. A cheque for the amount of the Affiliation + Licence Cadre technique fees<\/strong><\/li>\n<\/ol>\n

      3rd case: you manage a SAS or SARL<\/h3>\n

      1. Demande d\u2019Affiliation d\u2019association<\/a><\/strong>
        \nIndicate the name of your Studio along with the information requested for your Studio (phone, address, website, etc.) in the section labeled \u00ab Association \u00bb
        \nIndicate the name and the personal information of the manager in the section labeled \u00ab Pr\u00e9sident \u00bb
        \nFill out the section \u00ab Correspondant \u00bb if it is someone other than the President
        \nIndicate the name and informations of the Studio’s head teacher in the section \u00ab Cadre technique principal \u00bb
        \nComplete the form and attach a copy of your Kbis + a copy of votre CQP ALS diploma (if you have already received it. Otherwise, tick the box “En cours de formation” in the part labeled “Cadre technique principal”)<\/li>\n

        The studio that employs you will have already asked for the accreditation, so you have only to fill in the form Demande de licence Cadre Technique<\/a>.
        \nAttach a cheque for the fees corresponding to the license.<\/p>\n

        In all cases, each departement in France has slightly different accreditation and licensing fees. You must contact your Comit\u00e9 D\u00e9partemental to know your fees. You may find their details on the website: http:\/\/www.sportspourtous.org\/fr\/trouver-un-club-sport.html<\/a><\/strong><\/p>\n

        In order to find the “Comit\u00e9 d\u00e9partemental” you depend on, enter your zip code in the “Recherche g\u00e9ographique” field (beside the map of France).
